Photo Release

I hereby grant Sage Traveling, LLC and all business partners and associated websites (collectively known as “Sage”) the right to use, reuse, publish and republish my name, voice, likeness, photographs and/or other indicia of identity, and any information I provide about myself, in any medium now known or hereafter developed including any print or digital useage for promotional material or publication, alone or in conjunction with other material, without restriction as to changes or alterations, for editorial, educational, promotional and advertising purposes, including without limitation in connection with the solicitation of contributions and the furtherance of the corporate objectives of Sage or its business partners, and to use biographical and other information about me in conjunction therewith. There is no time limit on the validity of this release, nor is there any geographic limitation on the use of the photographs.

I understand and agree that all photos will become the property of Sage Traveling, LLC. I acknowledge that my participation is voluntary, and I will receive a $50 credit voucher to be used for future travel with Sage Traveling or Accessible Caribbean Vacations in return for signing this release.

I waive any right to royalties or other compensation arising or related to the use of the photos and likeness.
